Dialing up the ILS to 28R SFO(if not notamed OTS) even when it is VFR should put you on the correct runway. And with today's FMS's putting in an extended centerline will help out.

Prohibiting visual only approaches for heavys is probably another good idea.

Prohibiting visual only approaches for heavy's is probably another good idea, would not solve anything. Because the airlines today have the best avionics going. The correct use and using backups such as dialing in extended center-lines where no LOC or ILS is available and dialing in the ILS at all times will keep you out trouble. It is all about situational awareness, use all that you can. It is your paycheck!
