I would hazard a guess this would belong to Clay Lacy. He flew for United Airlines.

First line of report reads
'Constructed as a C-47B-50-DK'

This type (the Douglas DC-3) is just so prolific and was continual updated it frequently has difficulty delineating one from the other.
